This is caused by a failed connection to Quickbooks.  In the task manager, you may notice "SLQuickbooks.exe" in memory, but cannot find it on the task bar.  Killing the process is not enough, as Quickbooks keeps the failed connection, and will not allow another.  You must either forcibly shut down Quickbooks and SLQuickbooks from the task manager, or reboot your computer which ultimately does the same.
